GUIDANCE

The purpose of this survey is to discover differing conditions & problems & the enormity of these. The information gleaned will be used to prioritise seminars & veterinary help in the future.

It is not a one – off exercise but aims to be on-going. Therefore if information changes on a dog already surveyed – we ask you to mark the ‘update’ column so that the data is not used twice.
